Is she just drinking formula instead of eating?
If this is the case, I have heard a lot about this. Some children are born with an undeveloped stomach. One couple I know had to feed their son formula until he was 5. You might try to talk to a specialist and se if your daughter has un undeveloped stomach. You may also want to try some powdered enzymes and add it to her milk. I don't know if you can get it in a powdered form, but if you can it will increase her appetite a lot.

If you have tried everything and nothing is working perhaps a child psychologist would be your best bet....if she isn't eating enough to survive, she would not have lasted 3 months....let her be....she'll eat when she wants to....
