You will need to contact your insurance company to see what your deductible and/or copay is. If it's not covered at all, or under your deductible, you need to contact several testing facilities, and "shop out" the cash price.

Check your policy and it will tell you the deductible and co-pay. If the procedure is high, you will have to pay the deductible first then the insurance will pay its part and you will have to pay the balance. It is impossible to tell you a dollar amount without knowing what the procedure costs, what your deductible is and what the insurance company will allow. If you can't find the answer in your policy, call the company.

It depends on your individual policy. My Aetna plan, I didn't have to pay anything, but when I had Oxford, I had to pay a $50 copay. Some plans, you have to pay a copay, some you have to pay a percentage of the cost - after they pay, some plans you have a deductible - where you end up paying the full load - contracted rate, of course. (Meaning, you're paying what the insurance would have paid.)
