a bacterium called Rhizobium forms a symbiotic relationship with legumes. the bacteria fix nitrogen for the legume. if you see the roots of these plants they look like they have warts all over them. theses are nodules full of nitrogen-fixing Rhizobium.

Sorry..but these aren't "diseases". But, bacteria are involved in a) and d) for certain.

b) could be true by a stretch given that some extremophile archaebacteria consume metals (inorganic).
