I can't help you on the licking, but i work at a vet clinic, and i can tell you what we normally do on a first puppy visit. We generally check a fecal sample to check for intestinal parasites. 95% of all puppies are born with hookworms or roundworms. Not only is it unhealthy for the puppy to have these, but they can sometimes be transmitted to children. They will also want to deworm your puppy, even if they don't find any parasites, because 95% of puppies are born with worms, they may not find any sign of them on the fecal sample for various reasons. It is safest just to deworm them. Other than that they will examine him, give him any vaccines that he needs, based on what schedule he is on, and when he had his last vaccine. They will also want you to get him on heart worm prevention. It depends on where you live, but because heart worms are transmitted by mosquito's a lot of states in the US recommend having your dog on heart worm prevention year-round. That is what we normally recommend for MOST puppies. If they find something (like fleas, or an ear infection) on your puppy they might recommend something else as well. You want your puppy to be dewormed, and there are MANY different types of intestinal parasites that a puppy can have and different types of dewormer. A puppy can start on a heartworm preventative before 6 months of age without being tested. After that a heartworm test would be a good idea also. Puppies just like to lick, it usually doesn't "signal" anything

A licky pup is great! (except that puppy breath is kind of rank...)
As for the vet bills, I went ahead and put my dog on the Banfield's Wellness Plan. It covered all of his shots and office visits and cost about $15.00/ month. The nice thing was that it also covered the costs of having him neutered.
As with anything, it depends on where you live and the quality of care has a lot to do with the people working in your actual office. I LOVE my dogs vet. HATE his receptionist...
It is nice not to have to think about $$ each time I go in for a check-up (free anuals with the plan), or vaccinations. My dog is 3 now, so we go a lot less often, but I know that when he was little, we were always at the vet.

It is important for the puppy to have a fecal test done. If the puppy is 8-9 weeks old and hasn't had any vaccinations then he will need his first one..distemper/parvo/canine distemper/adenovirus-2. Unless you are in a lepto prone area there is no need to have lepto included in the first vacciantion.
A complete exam that checks, eyes, nose, teeth, heart, lungs, temeprature is a must along with a fecal test.

If you live in a warm climate, you need to have a heart worm test done and get him started on a heartworm preventative, so they will need to do a blood test for heartworm. They should also do a fecal to check for other kinds of worms. Other than that, don't let them talk you into a bunch of other stuff. They usually do try to do that.
