Drop by any United States Postoffice and pick up the application.

(only applies to the United States of course!)

You must fill out the paperwork, call you county offices to find it.
You need your SSN card, Photo ID( driver's license) and the registered copy of your birth from the county with the county seal and several 2x2 head shot pictures to use for the application.

They are about $60 and require a legal background search now, so it can take 30-90 days to process it. Unless you pay to get it done faster, that will cost ya. You can go anywhere in your state to get one. So go to a smaller town and it may go faster.

In the US you have to go to a building for the county level of government, iirc. I know where I live they actually opened a location in one of the more trafficked malls called County Corner, and the majority of my county's passports are now done through there.
