Nope.
No alcohol.
Some have caffeine.
It just a soda flavored with sarsaparilla root.
Their are many different recipes and a lot of micro brewers make it.
It's very good. Especially if it's brewed.

There is NO alcohol in Root beer, below is a standard recipe.
Root Beer Soda
Ingredients
2 cups sugar
1 cup water
1 tablespoon root beer extract
64 ounces carbonated water 1. Stir 2 cups sugar into 1 cup of water and just heat until all dissolves and is clear.
2. Add 1 tablespoon of root beer extract to complete syrup.
3. Place 2 ounces of syrup in a 12-ounce soda glass.
4. Add a little ice and soda to fill: 8 ounces of soda to 2 ounces of syrup. Stir only once so that you don't work out the carbonation.

There is no alcohol in root beer; it is a carbonated soda-pop product. The best I've ever had is called Virgil's (originally from England actually) and is hand crafted and contains all natural ingredients. Most root beers in the States contain corn syrup instead of sugar and artificial flavoring. The real ingredients list is below:
Virgil's Microbrewed Root Beer contains these key all-natural ingredients:
carbonated water
unbleached cane sugar
Along with these natural herbs and spices (including point of origin):
anise from Spain
licorice from France
vanilla (bourbon) from Madagascar
cinnamon from Ceylon
clove from Indonesia
wintergreen from China
sweet birch from the southern US
molasses from the US
nutmeg from Indonesia
pimento berry oil from Jamaica
balsam oil from Peru
cassia oil from China

According to Wikipedia:
Root beer is a beverage made from a combination of vanilla, cherry tree bark, licorice root, sarsaparilla root, sassafras root bark, nutmeg, anise, and molasses among other things. Each root beer has a unique recipe. Root beer constitutes about 3% of the American soft drink market. Many local brands of root beer exist and home-made root beer is made from concentrate or (rarely) from roots.

It's a flavor of soda (or pop or tonic I don't know what you call it) It's hard to describe the flavor kind of creamy (sort of) It is not alcoholic, it doesn't even have any caffeine.
My favorite flavor.
