How would I go about getting copies of my hospital records from childbirth and surgery? I am starting to have health issues that may have started several years ago and I'd like to have all the information together. I know the hospital name and can get addresses, but don't know the doctors involved and in one case I am fuzzy on the date. I think I am entitled to records by law, but I'm sure there's a lot of bureaucratic stuff to get through first. Any assistance would be appreciated!

By law they are your records & you can get them. HIPPA is something set in place to protect Ur information. You have to contact the hospital and sign a release of information form in order to have them sent to you. It would be easier if your doctor would put in the request. You could do the paper work in their office & they could have you files faster than if you were to request them. It's a hassle but it's a good system in Ur advantage I've had to do this when I moved to another state to keep in Pain Management. It sounds worse than it is. I hope this helped you.

You should be able to just call the hospital to obtain the records - as long as you were a patient at that hospital, they should have the records. I will warn you, however, that some places will charge a fee to duplicate the records for you.

you can usually go to their web site and order these documents online. Do a search for the hospital name and see if they can send you the copies that way - they just go by your name/social security # and it will cost you money though.

You will need to sign a 'RELEASE OF INFORMATION" form to obtain your records. Sometimes they charge and sometimes they don't. You shouldn't have any problem.

just call the hospital and request the info.they will,more than likey mail youa release of information form.fill it out and mail it back,voila,they will mail you your info,good luck
