there are lots of ques about the baby surname but if your seperate and have baby with someone eles do u have to use your married name or maiden name on the ceitifcate

you can give the baby any surmane you want, It doesnt have to be yours or the baby's fathers or anyone elses. you can choose whatever name you like.
As far as YOU go... on all 3 of my kids birth certificates it asks for my maiden name, not married name.

I was not yet divorced when i had my youngest, I put my maiden name on his birth certificate and gave my son his father's (my fiance's) Last name. HTH

You have to use your legal name at the time of the birth. That can always be changed later on the certificate. But they will ask for your legal name, and unless you changed it, your legal name is your married name.

Under most legal cercumstances you are required to use the father's last name. In rare cases you can have the mother's last name instead.

Since it is a legal document anyone that is named as a parent will be held responsible for child support and such later on.

You DO NOT have to use your legal name at the time of the child's birth. I did not.
