Easiest way to do this is to use the following phrase.
"Hey, do you want to grab coffee/dinner/a movie sometime?"
Use that phrase offhanded and just toss it out there so it doesn't sound like a big deal, like you would ask your best friend.
An even better way is to coordinate a bunch of friends(both sexes) to go somewhere like a bowling alley. Just mention that a bunch of you are going and see if she wants to go too. That way you're including her, not singling her out. You can go from there.

don't be afraid of what she going to say, just tell her how you feel, If you are like me you get the butterflies when you talk to a girl you like and you always find yourself not telling her what you want to tell her, but i have recently tackled this. I have found out that you will never know unless you tell her. It worked for me. good luck

After putting it in perspective, the worst thing she can say is........
no.
Being told no is part of life, so go ahead and ask and start living life to its fullest.

Dont be afraid to tell her how u feel!!!!!!!!!!! girls are really afraid to ask guys out. like me i never have and no 1 has asked me out yet!!!!!!!!!!!i have never asked ne1 out before and now it has ruined my life!!!!!!!!!!so ask her out already or else your life will be ruined and you will never become popular!!!!!!!!!!!!!!
