I think the guy's anger is justified. She should have been honest with him and allowed him to make the decision to marry her or not. One of the reasons for premarital blood testing was to make sure there were no hidden health issues. Unfortunately there is no sure-fire way to know whether what you're seeing is what you're actually getting aesthetically.
I think it's wrong in the same sense that a transsexual tells a man they are merely barren.
If the alteration is significant enough, be honest about it. Lies and half-truths have a way of coming back to haunt you.

Well honey I have 1 child and I love him more than anything in this world but I must say I call you very lucky. At this day in time kids wan't so much and they expect so much and if not raised correctly they don't appreciate the things you do for them. I have friends and family who are always saying they wish they could change there children. I have raised my son very openly and honestly, I wouldn't change one thing about my son, he knows i am very open minded and funny crazy and I love it and most of all he loves his mom. You can always adopt a child, or simply take a family member, or friends child for a day or a weekend (week) and you can see what you are missing.
