a 1400's painting called, tne next universe?

Question

It's a painting of rolling hills, distant houses on the edge of a lake, river. Hugh sun , cresent moon, 6 ponted stars, a man crawling along a stream, reaching into the next universe which looks like its on fire, maybe? with a big wheel in the sky?

Where have you seen it ?

It doesn't ring any immediate bells, but it sounds very much like the type of thing that Hieronymous Bosch (1450-1516) would have painted. I can't find a painting of his that fits your description though.
