I'm sorry that you have this problem. My nephew is diagnosed as ADHD so I know some of your problems. The subject is too complex to tackle in a short answer. At the risk of sounding simplistic, it may be necessary for you to not make eating a big issue and let the child feel more relaxed about it. You anxiety will transfer to him and make him even more tense.
I have watched Dr Tanya Byron on 'The House of Tiny Tearways' and I think she works wonders with children like that. She has written some books on the subject, one with the title of the show. If you can get hold of the videos, they are also very instructive.
Sorry I can't be more help, but Tanya might.

I had that problem and the only soloution is to stop the medication. I used to lose 30 pounds when i would take dexadrine and then gain it back when Istopped. and now that I'm 24 when i tell people i was idagnosed with adhd they laugh because im super mellow. I've never been able to really gain weight properly since. I am 6'3 and I weigh 145 lbs.
If you have cash I would look into montessori schools. That would help you stop the medication.
I think lack of exercise is the leading cause of misdiagnosis of adhd.
I think only 15% of the people diagnosed with it should actually be taking medication. Never forget how much money is made on this illness each year.

Vitamins & supplement drinks high in fat, protein & carbs. You can mix them with full fat milk & they can taste like a milkshake. You could also make your own fruit & veg juices. Apple, carrot & betroot juice is a hit with the kids, very sweet & high in nutrients. It's very stressful when your child will not eat but do try to relax a little, he will be fine so long as he is getting enough to drink & at least a vitiman supplement.

I don't think adhd exists i think its a medical term to give kids who are trouble and crying out for attention by having bad behavior. If something is really wrong with them mentally its not adhd.Thats just my opinion i don't think kids should be drugged up.

try not to make a fuss around meal times. I know its hard, but if you relax, and not make an issue of it, then he may eat without realising it.
Talk to your Doctor, I only did a part time course in Social studies, so am no expert

Why is he on medication at his age. get him to join an athletics club, play football, run round the park instead of changing his brain chemicals. That would tire him out and give him an appetite.
