An orthopedic doc is worth seeing. You might just need custom arch supports!
There is a series of three epidural shots which are used to treat this back problem. They usually work, but not until after the third shot. Ask you doc about this.
If these solutions don't work, there are other solutions, up to and including severing the nerve which is causing the problem. Keep talking with your doc, and don't give up!

I have Fibromyalgia, trigeminal neuralgia, 3 discs out in the cervical (neck), 2 out in the lumbar (lower back) degenerative disc disease, arthritis, carpel tunnel and sleep disorder, which actually is due to all the above. Anyway, I have been on all the medicines you have listed as well, with no real relief. Some take the edge off, but nothing fully rids the pain. Even had duragesic patches, didn't really help. The only medicine I take regularly is Neurontin for pain management. Otherwise, I do go to the Pain Clinic and have gone for the last 12 years. I usually have to go about every 5-6 months for either a nerve block in my head for the trigeminal neuralgia, or cervical and /or lumbar epidurals for the back. THEY HAVE BEEN WONDERFUL!!!!!!!! It really beats being doped up on a bunch of medications all the time. Some people get relief permanently with 1 or 2 SERIES (3 to a series) of epidurals, others like myself have to keep going. It still beats being on meds. The pain meds do not work well because it is more of a nerve ending disorder and inflamation. Sometimes they take an edge of, but the anti-convultant medications used to treat these diseases are better, that is what Neurontin is. Best of luck and hope you feel better, I know your misery, it isn't fun or easy. Try the Pain Clinic, don't be afraid. They are skilled at what they do and technology is very advanced.

Unfortunately I have been going through the same thing for the past 11 years after having an epidural with the birth of my daughter. I take ibuprofen 3 times a day, get massages regularly & have seen a pain manaagement doctor on & off. I also use a heat pack on it at night when I sleep & have tried muscle relaxers in the past. Other than that there is not much the doctors can do other than put you on strong pain medicine & give cortisone shots.

You should check this sciatica natural treatment http://cure-sciatica.info
Normally, I would not answer a question like this as my expertise is generally in medication but I had severe sciatica while I was pregnant with my 3rd child (after having had twins) and I had sciatica so bad that I was finding it difficult to walk. I tried massage therapy and physical therapy and could not take medication
I went to the chiropractor and the pain was instantly relieved. My hips and lower back were out of alignment because I had carried twins the year before. On occasion a Doctor of Osteopathy (DO) can also work but they usually only work on one joint at a time - the chiropractor will do your whole back which works faster. Anyone who does not believe in chiropractors will tell you they are quacks - don't listen.
I don't necessarily believe that chiropractors can cure things like diabetes but they can definitely relieve some types of back pain especially if a pinched nerve is involved which is usually the cause of sciatica. Try a reputable chiropractor ASAP!

I don't think that and epidural would cause long term back pain unless done improperly. Have you had Xrays or MRI? MRI will find source of most problems. It would be hard to recommend treatment without know the source of your problem. Could be muscular, spinal, a disc problem, or number of other things. Ask doc for MRI if you haven't had one.
Your Dr. can also give you literature on treating and preventing back pain. There are many stretches and excercises you can do to prevent back pain and injury.
