Don't visit them at their house.
You could make up that "famous" excuse many people use (even though it may not be true for many people), "I'm allergic to tobacco smoke - want to meet me at ___________for lunch?"
Other than that, just live with the smell you don't like. Bringing something to kill the odor would be too rude.
I don't think there's anything wrong with your being honest with the people and saying that the smoke smell is something that bothers you, and you'd rather visit somewhere else. Most smokers would understand that. Starting to shoot air freshener from a can in someone else's house wouldn't be great. It is their house. They don't mind the smoke. They may find your air freshener objectionable, and they would certainly feel insulted by your choice to just take it upon yourself to bring something rather than just being polite but honest with them about the smoke.

If you find it offensive you could always stop visiting. Failing that it would be rude to bring something to eliminate the odour. Why not suggest that you sit outside (if possible) so you can talk in the fresh air and the smoke won't bother you as much?
You can't tell someone to stop smoking in their own home.

Bite the bullet. It's rude to comment on the smell of someone else's home. If you don't like it, invite them to your house where you can make them smoke outside.

I'm very much anti-smoke but in this case I have to side with the majority of the people answering here... it is their own home, and it would be rude to carry any airfreshener or whatever. Some people really hate the smell of those, so using them in *their* home is not acceptable!

What would you do if it was BO, or smoke from a fireplace or cooking smells or pet odor instead of tobacco smoke?

Bite the bullet or don't visit. If they visit my house they go outside and better not throw those butts in my yard. I will be nice and provide an ashtray for outside use

Dont complain but let them know that the smell is evident, like you said that they are used to it. They may not be aware of it and I know if you were in my house and found ANY smell offensive I would rather you say something so it could be addressed and taken care of as soon as I could.

If you go to their place you have no right to complain to them, you can leave. Nothing you can bring will kill the odor.
