How to make good decisions? - learn the game (read the main books, know how to calculate odds, learn how to read other players, learn how to think on Level 2 and 3, etc.), watch how the winning players play, and practice, practice, practice.

The simplest answer is to tighten up your game. Play only the best hands and fold if the flop doesn't go your way. Don't chase after draws (unless you can do it cheap), and make sure to use the player notes function that is available on most sites. Mark down who plays loose, who's passive, who will chase, and everything else you would pay attention to in a casino. Another big thing is to play a table with the highest limit you can afford. At cheap tables, like $0.50/$1.00 limit hold'em, you'll see guys calling a twice raised pot under the gun with junk like Q6 suited. You might think that would help you out, but it takes a lot of the skill element out of the game. Play higher limits and the people are more predictable. This assumes you're good enough to make money, if not try reading up on basic strategy. T.J. Cloutier's Champion series are great.

play like a donkey. seriously, it seems that any time i've got a great hand there is ALWAYS someone out there that played s.hitty cards and totally sucked out on me.
Kinda like if you had A/K and went all in becuz you were short stacked, someone is going to call you with 7/3 or something and make a better hand than you. Never fails.

Don't chase if someone is betting high unless you KNOW you have a good hand. A pair of 3s when the flop doesn't give you a set is a great time to fold.
