I would at least call the vet and tell him what is going on. Fresh blood means that something burst, either a blood vessel, or a polep or something.
It could be part of the infection running its course, but that doesn't sit well with me.
Antidepressants? Jesus. Is he going to send her to a therapist, too? She's probably got one hell of a headache from the sinus infection!
You might want to think about another vet.

The first thing to do when you see blood is call the emergency vet hospital in your area and explain the situation. Perhaps the antibiotics have dried out her nasal passages and they are bleeding like chapped hands would, but you can never be too safe. In regards to the lethargy, if she doesn't feel well, she's going to be lethargic and antidepressants are probably going to add to it.
I have had the pleasure of taking care of many dogs, and I know that whenever I am as worried as you seem to be, I get dressed and take them to the emergency clinic no matter what time it is or the cost. That's what Visa and Mastercard are for. And I also know from a couple terrible experiences that not all vets know what they are talking about. So, if you are not happy with the care your little one is getting, definitely get another opinion.

Red blood would indicate that there is an open wound in her nose. Probably not a big concern. If you have concerns with the antidepressants you should call your vet.
I think I would give her a little more time and see if she improves at all. Dog's are alot like people, in that it will probably take a day or two for the meds to start working. If she doesn't improve, or gets worse, call the vet back and see what the suggest. My vet is usually very good about answering questions over the phone especially after a trip in.

I'd go to a second vet. She shouldn't be lethargic (unless she's losing a lot of blood) and shouldn't be on antidepressants for a sinus infection! Do you live in a cold climate? If so try making sure the air she breathes is humified (moist). Dogs like humans have a lot of blood vessels in their noses. I try and dry my laundry (especially towels) by hanging them inside in the winter. Saves energy and puts moisture in the air. Human get more nose bleeds this time of year too but if you're worried take her to another vet! I've never heard of a dog being given antidepressants for a nosebleed.

What is the name of the 'antidepressant' medication? I might be able to tell you why it was given.
Is she showing any other signs, like vomiting or diarrhea, or coughing or sneezing? (Sneezing due to the bloody nose would not be unusual.) Is she eating or drinking?
If it is a sinus infection, the antibiotics will take a few days to work.
If she begins to bleed profusely from her nose, you may need to take her back to the vet or to an emergency vet.
Other things that can cause this include fungal infections, and, unfortunately, tumors. Hopefully this is a simple bacterial infection and all will be well. Keep us updated.

Take her into an emergency clinic. Aside from dealing with the problem at hand (and helping you with your anxiety), they may be able to recommend a vet that you feel more comfortable with for a second opinion.
