The bone of the sinus is still not connected although the sinus is closed. I know that some doctors say operate to close it, some say do not operate. I have been to 5. It is often uncomfortable and tends to cause a runny nose and sinus pain. I have to take antibiotics at the first sign of an infection which is not so ideal. Any suggestions?

My suggestion would be to go back to the OS and take his advise, have the surgery if the prognosis looks better than what you have now. You didn't say how long ago this happened. But if you've seen five doctors it must have happened within the past couple of weeks. So if it's not going to heal on it's own, and your discomfort level is too much to continue with, then I would have the surgery. I've never seen anyone who didn't resolve to have the surgical repair. It's not something that happens everyday, but there is always a possibility of it happening when maxillary 3rd molars are extracted. Good luck!

I'm a dentist.

Dr. Albert, you are a discredit to our profession.

First of all, you don't put "Dr." and "D.D.S." together. It's either "Dr. Albert" or "Albert, D.D.S.". I'm ashamed to say that I see this mistake from dentists more than any other health professional.

Furthermore, it's "stitches", not "stiches".

You don't just "have stitches placed over the socket" in situations like the one described above, where either a large oral-antral communication exists or an oral-antral fistula has formed.

Instead, a fistulectomy is indicated, placement of gelfoam, and primary closure (with sutures) is indicated. The patient should be placed on proper antibiotics and sinus precautions.

Typically, these sinus communications are handled by oral surgeons, although some general dentists (such as myself) know how to handle them.

Apparently you don't.

THE EASIEST THING TO HAVE IS STICHES PLACED OVER THE SOCKET WHERE THE TOOTH WAS PULLED. IN THE MEANTIME, CONTINUE TAKING THE ANTIBIOTICS. YOU MAY CALL THE OS AND ASK IF YOU SHOULD DO SOMETHING DIFFERENT.
