He's probably having a very hard time coping with his "situation." and chooses to deal with it by lashing out plus he is in his teens when peer acceptance is EVERYTHING. Some teens choose to cut themselves or be promiscuios.

He might qualify for wraparound services (therapists or behavior specialists who come out to the home for a certain amount of hours a week to work with parent and child). Whoever his HMO or medical insurance is, call and find out the behavioral healthcare helpline or referral phone number and someone should be able to assist you. Tell them the child needs a psychological evaluation, and be ready to explain all of the incidents of rage and what happened. Look on the back of his insurance card.

If no insurance contact mental health services in your county or city and keep calling until you have an appointment made for a psychological evaluation for this child. If his behavior threatens his safety or others safety he can be involuntarily committed to an institution ("302'd") temporarily (not as scary as it sounds, kids often love these treatment environments).

Has there been any significant changes in his life lately? Like moving or changing schools. And how long has this been going on? With certain kinds of handicaps, a major change can trigger behavior like this and he may or may not understand why the change happened.

i know this sounds really harsh..but i would take him to a hospital to be admitted into an adolescent psych. unit
its not the most fun place to be..but my parents took me there and i was diagnosed with bipolar. he may get mad but in the long run i look back and it was worth it. since you are there inpatient they can observe you all the time..and try to figure out medicine for him and can diagnose him.
its not easy to go through but it will help him and you cousin will not be trying to help him alone..because there is only so much she can do

Has her doctor explained why this is happening, and what he expects to see in the future, If not those answers need to be addressed, and now, or they are going to have 2 patients in the hospital . help her help herself, support her in getting the answers and then dealing with what must be done, and good on for you for caring

Apparently the Dr. was not concerned. She needs to contact the Dr. again, or find another one, or maybe a psychiatrist. He may have a chemical imbalance of the brain which would be causing this.
