I have this problem that is ruining my life. Im afraid to do anything for fear of being embarrseed. around everybody, including my family. I wont go out with my friends, im terrified to go on dates. the whole deal. I need some advice.

Get over yourself, and stop worrying what others think of you. Go talk to someone about it. Your social phobia will completely ruin your life, as you see it is now. It's not that uncommon. Keep a journal of how often you think you'll be embarassed, and how often you really are. You'll see that you're greatly blowing this out of proportion.
