I simply wish to scan this document into a word processor, and add, delete, and otherwise make changes.

I have made several attempts with this HP all-in-one printer using its buzzTwain OCR software, but the document scanned to msword is unreadable.

Can someone instruct me how to successfully scan a doc to msword, and edit the text?

On one ocassion, the doc was successfully scanned to word, but when I attempted to edit it, a "format picture" dialog box appeared. HELP

It is a formal document that has my signiture.

Needs to be Scanned to OCR using Proper OCR software. In most cases, on OCR conversion has about a 93% success rate, and so some will convert as garbage or an incorrect letter or word

The success you experience with this will highly depend on the type of OCR software you are using and also the quality of the material which you are scanning. I've used OmniPage OCR software in the past with excellent success on standard font text, but as soon as graphics, non-standard fonts or other non-text items become present in the material, the OCS gets confused if human help isn't provided. The key is to use the OCR software to indicate which portion of the document is text (ie. printed in standard fonts) and indcate the portions that should be excluded from the OCR (eg. signatures, graphics, borders, etc.)
