You could just try to be simple and get him something he really likes (if he's into music, an album, if he's into sports, a memorabilia of his favorite team, etc.)
Believe it or not simplicity is always the best way to go. Never over do it.

Buy him a super cool sweater, and then in the sleeve, maybe inside the cuff stitch a little heart inside with a secret message embroidered. It's super cute.

They have the cutest stuffed animals this year. You pull them apart and a magnetic nose attaches them back together and then you hear a kissing sound and then it says I love you. They're so cute.

If you can cook..prepare him an intimate meal for 2. If not, plan to spend the day together doing something you both enjoy. The most important thing is to share "together" time. I think the effort would be appreciated.
