www.fafsa.ed.gov
You can fill them out online which is much easier!
Or you can go to your campus financial aid center or student center and they should be able to provide you with the forms

You can fill out the FAFSA online at www.fafsa.ed.gov. You need to do that to get any federal aid (Pell Grants, Loans, etc). The school will also get the FAFSA and will use that to determine any school grants.
Usually scholarships have separate applications through the school's financial aid office

Go to www.fafsa.gov. I believe you can fill out the forms online.
Go to your counselor or the financial aid office and ask for the form.

Please do NOT go to www.fafsa.com -- this is a for-profit site.
The FAFSA is free and the URLs you want end in .gov (as above).
