Tools > Letter and mailings > Mail Merge > Choose Labels and click Next at bottom the bottom-right

Click Label Options, choose your label # 8462 > Ok

Use existing list > Click browse and change the file type to Text (csv) and direct to the file.

Use Word's own Mail Merge. You may have to suck the cvs file into Excel first, but there may be a way to read it directly. To find Mail Merge, click the Tools->Letters and Mailings->Mail Merge Wizard menu item. It's not a snap, but once you have it set up, you should be able to use it again and again.
