The Vatican

Maybe I should not answer this question because I do not have statistics at hand. I recently saw a news article somewhere or another on this subject. If I remember right, the two countries with the highest proportion of Catholics were Italy and Poland at over 90%. I was surprised to see that Ireland and Mexico were not at the top of the list.

In case you were wondering, my family is Polish Catholic and my wife's family is Irish Catholic.

Besides the Vatican I would take a guess and say Nicaragua and the Phillipines

I pulled this from wikepedia -
In Africa, it is most dominant in the central part of the continent, while in Asia, there are only two majority-Catholic countries: the Philippines and East Timor (due to Spanish and Portuguese missionaries).

Besides the Vatican, I believe it's Malta
Every villiage no matter how small has a huge church.
80% of the population go to church more than once a week, often popping in to say a prayer every day.
Divource and Abortion are illegal.

Sounds pretty Catholic to me!

Probably some of the South American nations.

They, for example, have an extended holiday celebration that goes from All Saints day pratically up to Easter.

They have traiditions that occur after Christmas.

They also have very good family values and probably have a low divorce rate.

They also still dress up in their Sunday best.

Vatican city.

In terms of % because it is a separate country from Italy and you can bet just about everyone there is Catholic with the exception of a few tourists, there are many priests, cardinals and of course the pope himself.

Mexico has the highest Spanish population, so that would be my guess. Other countries have lots of Catholics, but lots of 'other' Christians too.
