Up to you to chose where you want to start . But you must gradually increase your intensity according to your heart rate .A proper record of 1.Frequency 2.Duration 3.Intensity 4. Max Heart rate reached.

It is good, but it does mean that the exercise your doing, is for that rate, It depends on the source of exercise your doing, the harder you work out the faster it will beat. The average heart rate is different in everybody, some it will beat faster than others,so 165 is a good average.

I assume you mean heart rate? I would expect your relaxed pulse rate to be about 70. Doubling it with aerobic exercises is good. 165 bpm is perhaps a little high if you are sustaining that over a period. The true test is how long it takes your heart to drop from that peak to its normal bpm's when you stop exercising.

It depends how you feel and your age, I am 40 and keep a heart rate of 175 for 2 hours on the treadmill. this is about 98% of my theoretical maximum. I am used to it and it does not tire me out, so it is fine and makes my heart stronger. If you don't feel fine and can't finish your workout, it is bad. You will do less exercise and if you push yourself, could have heart problems. the key is making it routine and feeling comfortable in order to avoid problems
