For good examples of documentary photography, look at National Geographic or Life magazines and books. The camera is not as important a factor as the photographer.

No... most possibly but you can create a photograph that is not a documentary... it can be pure art.

All photos are documentary. For legal purposes they must be able to verify time, date, and location.

In the large sense of the word, Yes, since you are documenting what is in front of the camera.
