If people are stepping back from you when you talk, you probably are in the advanced stages of perio disease. It gives a horrible odor, that most people who have it don't notice. I don't know when you last went to a dentist, but you need to go now. Symptoms are bleeding, red, swollen or inflamed gums, some say they taste infection or something bad in their mouth, one or more teeth may feel loose, some teeth may even appear longer. Any one of these should be checked out by your Dentist. You may brush twice a day, but how well do you brush? I've found that most people don't know how to brush correctly, or effectively. If you don't brush right, the teeth and gums aren't being cleaned, no matter how long you work at it. Which leads to the dental problems that send people running to the dentist in pain. If you want your problem solved, go see the Dentist for a through cleaning, exam and treatment plan, and ask them to give you correct brushing and flossing advice if it's needed. Don't feel like you are alone or caused this, sometimes there are other factors besides brushing that are involved in developing this. I hope I've been on some help, good luck!

Bad breath doesnt just come from the mouth. you could have a throat or sinus problem. I would see a nose and throat doctor if it is that bad.

you seem to be having healthy and proper oral hygiene. but you might need to brush three times a day immediately after meals and use stronger toothpaste. and if it continues, i think some of your teeth may need to be removed. you need to see a dentist buddy.

are you brushing your tongue and cheeks, as well as the roof of your mouth as far back as you can? try a non alcoholic antibacterial mouthwash too.
