Bad breath (or halitosis) can be caused by many different things. Bad Breath can result from bacteria that builds up in the mouth. Your teeth attract bacteria. It attracts bacteria by food getting trapped in between your teeth and the food particles that stick to your tongue. Bacteria adheres itself to the foods surface. Bacteria causes food decay and it's the bi producers of decomposition. Your teeth can also start to contain plaque if you treat your teeth poorly. You can treat the decay by drilling it out. You can prevent plaque in the same way you prevent stinky breath which is brushing your teeth regularly and adequately. You also can use mouthwash to kill the bacteria because of the chemicals contained in it and the fact that the smell of it makes your breath smell good.
Bad breath can also be formed from different diseases. One of the reasons that can cause your breath to stink is infections in the lungs and sinuses. Infections in the lungs and sinuses can make your breath stink because all the bacteria can lead up to your throat and all the bacteria can cause a bad smell.
Diabetes can also cause bad breath. Diabetes can cause bad breath because some of the foods that people eat. The food that causes your breath to stink is low-carb foods. The reason low-carb foods will make your breath stink is because there is a chemical called Atkins which is an effect of bad breath.
Disorders of metabolism can cause bad breath. Slow metabolism can also cause bad breath because all the food is still stored inside your body. That is why it has a fishy, smelly, foul odor. The reason why it has a odor is because it comes and may be hard to diagnose.
Some foods that can make your breath smell bad are onions, garlic, and other different spices. The chemical or ingredient that can cause your breath to stink can be the oil in your food. Onions have a chemical which called allin and allinase. The chemical sticks to your mouth cells and that is what causes the bad odor.

Usually it is due to poor hygiene. However there are some respiratory problems that can contribute if I am not mistaken. As far as hygiene goes, it can be more than not brushing your teeth. There are people who brush properly 2-3 times a day and still have problems. This could be due to poor dental care that leaves food between the teeth or a build-up of other substances resulting from not getting all food particles out. Bacteria is a big issue as well. It helps digest food but doesn't always smell nice. A dentist can always assist in fixing bad breath.
