A man drops his banana peel, someone slips then accidentally throws their breefcase and hits some one in the head an kills them.
OR
An evil leapercaun eats a golden coin (by mistake, it was in his chocolate) and then a man who NEEDED the chocolate coin to trade with the giant wildabeast so he wouldn't eat him alive, and the wildabeast eats him. THEN the wildabeast chokes on a bone and dies, and a dude going to work thru the enchanted forest trips over the wildabeast's dead body breaks his own neck. THEN a search crew goes to look for the man in a search helicopter and it crashes and all the people in the helicopter died. That would SUCK!
