More specifically, a girl is out for a drink with another friend, or working out at the gym. She doesn't know too many people/somewhat new to the area. Guys will stare at her, say she looks "real" or "hot" to each other. But no one asks her out. Why?

The more attractive a woman is, the more intimidated some men will be.
A sexual attraction can be embarrassing, can cause a state of inferiority or mild psychosis, and prevent men from taking action. Still, it's cowardice.
Very bad advice by Mr. Smooth below, saying that you should make the move. Totally wrong.

Maybe you come off as a *****. If you don't know many people, perhaps you are being shy and if you truly are hot and you act shy, you'll look like a ***** who thinks she is above everyone else. Let it be known that you are having a good time, look like you are a fun person. You also need to look approachable. Headphones at the gym and having a drink with a friend don't exactly scream, come talk to me. Maybe you should make the first step. Ask for advice on how to mix up your workout routine or incorporate weights and definitely say something flattering about his cardio endurance or muscles. At a bar, guys are much more likely to approach a group of 3 girls than just 2. If you see a group of guys, invite them over with a cute little wave or go to them.

When a guy checks out a girl, it doesn't necessarily mean he wants to go out with her. Sometimes guys are already in a relationship but still look at other beautiful women. Nervousness is often the issue also. It's hard for many guys to approach strangers and ask them out.

they don't have the balls....the better looking a woman is, the less likely they think their chances are getting a date. Make eye contact with them, smile, and say hi. This let's them know they stand a snow-balls chance in hell of you saying yes if they ask....
