Tell your boss/supervisor. It's their concern because it's affecting your work performance, and possibly offending customers as well. The boss can deal with the situation anonymously.
Consider the possibility that your coworker may be suffering from psychological problems. Often personal hygiene is difficult for those who have depression, for example.

I would consider leaving a basket or little tote bag of hygienic supplies, such as deodorant and shower gel on his/her desk after hours. If you do it when no one is around, then you'll be able to send a very strong hint to this person without offending them face-to-face. Also, it could be very embarassing to get other co-workers or the supervisor involved.

Go to the human resource person and bring it to their attention.Or just tell her that she may want to go check in the bathroom, because there's a smell.There's nothing like odor from there...be bold, get to that odor one way or the other.......
