You should definitely read the Outline of Coverage provided by your insurance company. It will detail exclusions, as well as coverage.

In many states, like Connecticut, the State mandates coverage for maternity in group health plans. If you are still unsure, contact your state's Department of Insurance, or your Human Resources Department.

Sometimes it's included with a waiting period. Sometimes it's included without a waiting period, and sometimes maternity benefits are optional or not included. You will have to check with his employer, or call the number on the back of your insurance card, to ask.

Probably, but double check for sure. Some employers skip that rider to save money.

Did you thoroughly read the insurance plan so you know what is included?
