If you've been going to her for so long, it would be nice to give her a chance to make things right. If you know exactly what it is that you aren't happy with, ask her to fix those points. If you're more like me and can't explain the difference between layers and texturing, tell her that the cut hasn't been growing out the way it used to and that you want to talk first to see if you two can make it look the way it used to. If she's unresponsive, negative, or just gives you yet another unsatisfactory cut, simply make your next appointment somewhere yet.

If you are unhappy, say so. Be polite, maybe ask her if everything is ok. She might be going through something at home and not realize it's affecting her work. I have been through this with my hairdresser too. I was really as nice as possible about it but I told her I didn't think she was paying enough attention to what I wanted done. She apologized and has done a much better job since. That was about 3 years ago.

Isn't it funny how difficult it is to leave? It's literally like going through a divorce.
I divorced my barber several years back, and although it took a while, I finally found a miracle worker.
I don't know what to tell you. The logical thing is to mention it to her as tactfully as possible. I can't put words in your mouth, but I'm sure you can come up with something. Just prepare yourself ahead of time.
But if you do and nothing changes, leave. It may just be hair we're talking about, but still! I used to pay $10 and now I'm paying $42. It's totally worth it for the peace of mind.
It may take you a while to find someone good, so start gathering names now.

You don't want to be rude since she's the person responsible for how you look! But. . . it would be reasonable to say something about the fact that you just haven't been as pleased with the results, lately. If there's something specific you miss, you could just put it this way. "you know that thing you used to do, could we do that again? I liked that." If you address the issue and nothing changes, it would be acceptable to leave. I know it's hard but. . . it's probably better to go shopping for a new one now than to have a huge blowout later, like I once did over the fact that I got given bangs without asking for them.

You can talk to her about it and if things don't get any better find a new one.
You can just tell her that the results on your hair haven't been the same as they used to be.
