Well for the morbidly obese patients, the penis would appear exceedingly small. Add that with the fact that men aren't as large as what is usually seen on pornography dvd's and you have the question you asked.
Women's vaginas are custom made when a woman is created. Becoming fat does not shrink it. HOwever it will make it harder to get to, depending on how overweight you are.
