I had very serious acne problem too and was ashamed to wear low back or spaghetti top as the back of my skin will be expose. My problem was solve when I use Clinique Body treatment spray. It costs about US$18-19 but in Singapore the product was discontinued. You might find it in States, Europe or Canada.
Now I am using Dalacin T Lotion from the doctor. It also helps control and prevent new pimples from forming and once a week, I will scrub and mask my back. Any facial mask and scrub will do fine for the back but try getting rougher grains from scrub as the skin behind will be much thicker.

I've had the same problem, and I've tried all of the things people mentioned including a ton of different medications, creams, antibiotics, etc. I would still have it if I never found this solution to my back acne: I go tanning 2 or three times a week. That's right, tanning. I decided to go tanning in the winter one year because I was going to Florida for vacation. I just wanted to look good by having somewhat of a tan. Well, after I went tanning, my oily skin started to dry out and my acne started to disappear. I could not believe that something this simple and painless could cure my oily skin and bad acne. Now, I go tanning on a regular basis, usually 2 or 3 times a week. When I first started out, I only went 1 or 2 times per week and that still helped out perfect. The only reason I go more often now is to stay dark tan. It's just something I enjoy doing. So if you go 2 times a week, the most it would cost you is $16 or much less per week. I don't think you can compare that price to clear skin! I hope you try it and I hope it works out for you too. Good luck!

I've used Proactiv before, and it didn't work very well on me. I'm not saying it won't work on you, but it's way expensive ($40 a month) for the very little you get. Back acne is probably different than face acne, but I do know anything that has a grainy substance in it helps cleanse and exfoliate pores. I use St. Ive's Apricot Scrub.
But I would seek professional advice from a dermatologist.

As the skin on the back is thicker and stronger than the facial skin, the normal facial skin care methods will not be effective. You need stronger skin care methods to treat back acne. Combining two or more treatments of facial acne is the most effective way to treat back acne. Clean the skin with a gentle exfoliant. Then apply alpha hydroxy lotion and let it dry. First, try switching to a mild detergent(no fragrance)and no dryer sheets or fabric softeners. Then Neutrogena makes an all-over body wash containing salicylic acid. Also,Walgreens and Walmart have generic versions of Proactive that run about $14-19 bucks. And they really do work the same.
