You should also contact all 3 credit bureaus and have a note put on them, saying no new credit is to be opened without a verbal verification from you. So if someone tries to open credit the creditor will have to contact you to verify that it is you.

www.ssa.gov
and apply for a new social security card. and tell them your old one is last.you can do the application via internet.
