It means that either a weapon was used in the commission of the crime or the victim suffered serious bodily injury as a result of the crime. Either way, the offense is enhanced if the charge is "aggravated" which means the punishment is harsher.

Usually it means a weapon that can cause serious bodily injury or death was used. Sometimes it can be from a gross deviation of the usal caution used by an average person. But mostly due to weapons.

Assault???? In Canada, under the Canadian Criminal Code (in force throught Canada), there are three basic levels of assault - Assault (simple as a real or perceived, threat); Assault with a weapon or causing bodily harm; and Aggravated assault - serious injury requiring medical attention.
I suspect, if t you are in the US, it is similar.... Aggravated assault has the highest penalties.
