There is a 1040EZ and a 1040EZ-T - they are two separate forms, and each can be downloaded from irs.gov. If you are filing a very simple return, then you're looking for a 1040EZ. If you are not filing a tax return, but are eligible for the telephone excise tax rebate and are just filing to get that, you use the 1040-EZT. If you file a return with a 1040EZ, 1040A, or 1040, there is a line on the form to claim the telephone tax rebate, and you do NOT file a 1040EZ-T.
