ALOT of fluid. Why does the procedure hurt so much even with a topical anesthesia?

The reason it hurts is because it was only a topical anesthesia you had. Topical means exactly that, on the surface. Thus the purpose of that anesthetic is to numb up the skin where most of the pain receptors are located. This way when they use a much larger needle to go deeper to aspirate t he fluid it won't hurt on the skin as much.

However, you will still experience pain as there are pain receptors deeper on as well although not as much as on the surface of the skin. Most of the pain is due to the inflammation and stretching of the knee - removing the fluid will help reduce this and the use of anti-inflammatory medications will help further.

Usually when I do this procedure, not only do I provide topical anesthesia, but also go deeper and infiltrate the path down to the joint space where I will be aspirating with the larger needle. However, it is perfectly acceptable and within standards to just provide topical anesthesia as the administration of the anesthetic itself is painful as well.

because all they were able to anesthetize was the skin...there are nerves all around the bursa (where they removed the fluid from) and the knee cap and adjoining tendons and ligaments...i had that procedure done a couple of times i feel for you it is not any fun at all, follow your doctor's instructions to the letter, and you will get better sooner than later...i was never any good at taking instructions from ANYONE and now several years later suffer some consequences

Because of the nerves in the area. The same goes for elbow.

its typical for you to ache after a surgery. i mean, if you dug into someones knee, wouldnt you expect it to hurt?
