An Incident/Offense Report is a standard form, although may somewhat vary state to state, which reports the facts provided by a complainant. Normally, the initial responding officer writes the report. Should the officer make contact with a complainant, the complainant will provide their side of the event and sign the report. The bottom of the report will have the investigating officers name.
The specifics on an I/O Report include:
Time/Date
Location
Environment
Suspect or Defendants Name
Witnesses
A narrative providing the basic information provided by the complainant or victim.

If you mean on Yahoo answers, someone who read something that you wrote found it offensive in some way and checked 'report abuse'. They selected a main category and then could add a description if they choose. I believe Answers staff reviews the complaint before an email is sent to the offending user as a warning.
