The plastic surgeons in Vietnam are VERY cheap. But I don't think that is exactly what you are going for.
I would suggest looking into the public or government to find where the cheapest ones are. They usually can give you some good info.

In the states, I'd say Florida has the cheapest because of all the older people living there. There's more competition. Insurance payment depends on whether it's genuinely needed or not. For instance, you can get an eyelid lift paid for by insurance as long as it's obscuring your vision.
