Is their some question of your gender? Because I have no problem being address as "ma'am" or "madame". Maybe you should take to wearing more feminine clothes?

I am a server and never refer to both men and women as "Guys" I find it very annoying also. A lot of people compliment me for not saying that. I am also a trainer and I try to teach everyone else not to say it unless it is a group of guys.

I think the word 'guys' is leaning more and more in common usage to be synonymous with 'folks'.
How's it going? What's up? Hey. Dude!
All things my grandparents thought was horrible English. They are right technically speaking, but languages evolve over time - sometimes in front of our eyes.
I had a friend invite a friend of his up from Kentucky a few years back. Everything was "You boys..." or "Boys..." or "... you boys ..." That's how it was were he grew up and was a little odd at first. By the third day of his visit however, we all found ourselves saying it too - just had a cool ring to it.
If you find it annoying, understood, but I wouldn't take it personally; I seriously doubt its meant as an insult.

It's just slang I guess. I used to do it all the time and then I tried to get out of the habit of doing it and happened to move to "ya'll" when I worked at a BBQ place. Then I became conscious of that and started using "everyone".
Most of the time I didn't realize doing it, so I'm sure other people don't either...it's not meant to be offensive.

Yes, it is very colloquial and base. As a woman, I don't appreciate being addressed as "you guys". I never was a guy. I'll never be a guy.
Pehaps something like "you folks", as in, "What can I get you folks?". How about "What can I get everyone today?" or "Would you like to order now?".

I find it incredibly tacky when a group of women are addressed as "guys".
The last time this happened to me, I was with my mother, aunt, grandma and older sister. All of us VERIFIABLY WOMEN, and when the waitress said "what can I getcha guys" I said "no, not since the operation" and she said "whut?" And I said "No, we're not guys, not since we all had our operations!".
"oops, sorry hon!" is what we got after that.

it was meant to be annoying it leave an opening for the dumb girls to jump in and say their not guys~it is a conversation starter that is just one of the new language barriers that are slipping past the guys to get to the girls~in America we call that in your face

I think that depends on where you are. Here in the South, it's often "y'all," which is much less gender-specific. However, whenever it is done, it's probably just a time-saver if the whole table is being addressed.
