It needs to be uncovered in order for form a crust properly. If you cover it while baking, the bread will steam instead of bake.

It depends on what kind of pan you're taking about (and what kind of bread you're making). There are pans made to be baked with the lid on. It creates a thin crusted square loaf similar to the sandwich loaf you can buy at the grocery store. The link below shows an example. If you're using this pan, be sure to use an appropriate recipe or you could damage the pan or make a mess of your oven. Don't just slap a lid on any old bread pan, either.

If you are baking the kind of bread that requires a covered pan the answer is of course yes.
The classic case in point would be a Pullman loaf of bread which requires a Pullman loaf pan.
An uncovered loaf will create a more domed appearance- asuming that you didn't over-proof it.

should be left uncovered or else it will not bake properly. When it is open, you will get a good loaf of bread with a great crust, not too dry not too moist, just right!
when you cover it up, you will end up cooking the dough in its own steam and you may end up with a disaster... or with steamed bread!

It would not brown. Bread is very forgiving, it would not hurt it very much.

No cover, if you want to know what would happen, it will not rise. Try it...
