Call up the IRS at 1-800-TAX(829)-1040 between 7 a.m. - 11 p.m. local time. Tell them your question is about non-receipt of 1099, that you never received the 1099 from your last company. They should be able to direct you as to your options and help you from there.

You should also be able to use the last pay stub that you got from them, but have you tried just calling them, and asking where your form is?

You would file a schedule C or C-EZ, just like you would if you got a 1099. Show your gross income, and whatever associated expenses you have.
