Trying sitting or lying with a raquetball or a tennis ball against your back and roll back and forth over the knots. This should produce a self-massage to get them out.

I would first try a warm bath (Epsom salts helps) to relax the muscles. Before you get in the bath, I would take some Ibuprofen or Naproxen to reduce the inflammation. After you get out, lay a rolled towel under the sore part of your back and move the roll to other sore places as the pain subsides.
Otherwise, go to a chiropractor and have some laser acupuncture. Works like magic!!!
