Even if you think of a medical excuse, you would have to find a doctor who would unethically provide a diagnoses. I would assume your boss will need verification.
Don't try using the fear of flying excuse. If your company frequently or periodically sends you to different destinations, this would obviously hinder that.
I would suggest you find out what area of the middle east it is you will work in. If it's not a dangerous area, just go for it. It may be an adventure and a good learning experience.

I don't blame you, I wouldn't want to go either.
I think you'll struggle to come up with a medical condition for this. They may ask for a medical note! Why not just tell them you are scared to fly?
Alternatively, miss the plane accidentally on purpose.
Tell them you won't go unless you have the proper protection and by that you mean a 4 man squad from the SAS with you at all times.

Racism. Claim to HATE Middle Eastern People.
The world is so PC these days, that if you're fired for that tack, you can sue for being fired because of your political beliefs.
Alternately, claim to be pregnant.
Better yet, just tell your boss in no uncertain terms that you are NOT going, as you like being alive. This assumes you have a brushed up resume and an active and productive job hunt.
Finally, spend all your time talking about the "Arab Hookers" you;re going to go after. Make it clear that you intend to spend most of the time pursuing vice and sin, in other words, blowing off the whole affair.
Or you could go for the simple route. Mail in your Passport to get it renewed, so you can "Get a new one before the RFID tags are mandatory." No passport, no travel. Claim that you sent it out just before you found out about this trip, but only remembered you'd done that when you went looking for it.

could be a little late to get started on, but you could have a precarious pregnancy and the OB orders no air travel.
You could also develop religious or personal aversions to taking recommended innoculations for the area, and thereby be advised by your Dr to not travel to those areas.
Perhaps a mental illness requires institutionalization?
Maybe a drug habit puts you in rehab?
Is your work going to be in a particularly dangerous area? Hopefully it's not as crappy as you fear. It might kinda be interesting. Good luck.

High Blood Pressure is a good one! The only way anyone can tell is if they've got a BP monitor to hand!.....Oh and you may want to say you've got tinnitus( ringing in the ears) and a headache to back it up!

An ear infection would mean you couldnt fly as you could damage your eardrum.My partner found that out the hard way.

I don't think you can make something up as they would probably ask your doctor to provide confirmation. They can't force you to go abroad surely? Only thing I can think of is a phobia of flying!
