You should contact the Embassy of the country you want to visit, they will tell you exactly what they will need from you, how much it will cost, how long it will take, etc.
It's really different for each country, and depends on what nationality you are.

Before your planned journey allow plenty of time to get the relevant visa. Book airline tickets at least three weeks in advance. You can better plan your air travel through online booking on Internet.
Don't pack your passport in your rucksack - keep it in your hand luggage with your tickets. Try and memorize your passport number incase you lose it! Write down your passport number, addresses of friends and family, important phone numbers, ticket numbers etc in a small address book. Open a bank account in the local bank if you are likely to stay for longer periods. You can apply online for passport or visa for any country by clicking the link below.
