Urine is usually foggy, medically referred to as cloudy, when it is concentrated. This is normal first thing in the morning. The most common cause for it to be foggy, at other times, is usually due to not drinking enough fluids.
However, there are also other causes for cloudy urine (certain foods, infection, STDs, vaginal issues, prostate issues, etc) so if you are drinking plenty of fluids, you should see your doctor. He will run a urinalysis and, if needed, other tests.
