Well, it's ok to stick to the basics.
1. What is the nature of God to you?
2. How do you feel about Jesus Christ?
3. What do you think about life after death?
4. What sets you apart from other denominations?
5. Who is in charge of your church?
6. How do you pray, and how often?
7. What day do you worship?
8. What book or books are considered scriptures to you?
9. Does your church do missionary work, and why?
10. What makes you happy to be a member of the Church of Jesus Christ of Latter-day Saints?
Now if you are doing a survey with non-Mormons about Mormons, maybe try these questions below.
1. Do you know who Mormon was?
2. Do you have any friends who are Mormon?
3. Do you know the whole name of the Mormon Church (The Church of Jesus Christ of Latter-day Saints)?
4. Do you know any famous Mormons? If so, who?
5. Have you ever talked to Mormon missionaries?
6. Have you ever seen Disney's "The Other Side of Heaven"? or any movies featuring Mormons as main characters?
7. Did you know Elvis read the Book of Mormon?

Why do you think you must keep commandments to be saved? Isn't that by works instead of grace?
Why do we need new prophets and revelations when all we really need, the grace of Christ, has already been revealed?
What happens to those who are not a part of the LDS church?
Do Mormons think they will become gods?
Doesn't the story of the thief on the cross prove that salvation can be instant apart from works?
What is Family Home Evening?
Why don't Mormons believe in wearing the Cross?
Do you believe that the communion literally changes to blood and flesh?
Why do you use water instead of wine in the sacrament?
What are your services like?
How do you baptize?
Why don't you drink tea and coffee? Is coke allowed?
What is the Latter-day Saint practice of fasting?
What is "home teaching"? Is it like home schooling?
What does "sealing" mean?

Why doesn't the Church take action against some people with terrible sins?

What is the difference between Holy Spirit and Holy Ghost? I thought the Holy Spirit was the actual love of God and Christ in our hearts that we receive upon conversion.

Where does Joseph get the name for the "telestial" kingdom? That's not in the Bible.
Why do you respect Adam, the villain who wrecked God's plan? We'd all be happier in the Garden of Eden.
Why don't you accept the Biblical doctrine of salvation by faith alone?
Isn't your emphasis on works a departure from historic Christianity?
What's this about receiving grace only after "all we can do" (2 Nephi 25:23)? Doesn't that mean we are saved by works?

Doesn't the Book of Mormon say that dark skinned people are evil and that they turn white when they repent? Why doesn't that happen today?
Were blacks denied the Priesthood because of a verse in the Book of Abraham?
Is it Mormon doctrine that descendants of Cain cannot hold the Priesthood? Does the Book of Abraham provide the reason why blacks couldn't hold the priesthood?
Is black skin a sign that people were unworthy in the pre-existence?
