Had it been a longer layover, I would give you various places that you could in fact visit but my honest answer is this- stay close by. Traffic jams are incredible and more so there is lots of heavy construction to repair the broken highways. It may take as long as two hours to travel just a few miles. If you want to risk it by all means but personally, I would not do so.
I travel much and out of San Juan so I understand why the layover may be a concern. It's not an airport I like to be in myself but it would be worse if you missed your plane. Flights may be limited and you may have to stay over night- which means staying in an overly priced hotel near the capital for just a few hours since you will NOT be allowed to stay in the airport overnight. They have no security.
Also, there are terrible lines to get into the airport. And you may not find a taxi right away depending on the season. Public transportation isn't reliable either despite the location. There is rarely anything to see within walking distance either- save for a parking garage/ the highway/ or the tower nearby.

Old San Juan is very nice to visit. It is nearby but traffic is a problem. Unless you are sure of your timing and have an option of easily taking another flight if you miss this one, I would recomend you stay at the airport. Five hours is not enough to venture out if you cant risk taking a later flight. If you do get to visit Old San Juan I am sure you will enjoy the place. Take you camera!!!
PS: I live in Puerto Rico, and I can assure you that trafic can be the only mayor problem you can have If you venture out from the airport.

you could easily spend 2 hours or so in Isla Verde and be back at the Airport on time, go to the beach, or a casino there are several in Isla Verde, tourist shopping, did I mention the beach? take a cab, the airport is right in Isla Verde, I wouldn't recommend going into Old San Juan, although it's beautiful, and worth seeing and experiencing, the people above are right there is just not enough time to go there, and be back in time for security, because of the traffic. but the beach wouldn't be a problem.

Everything the have told you is fine.. but please... BE AWARE... the traffic jams in PR are a huge problem... especially in the Carolina area where the airport is located (I used to lived like 5 minutes from the airport and I know this area very well)... plus the airport is always very crowded and the lines to get throught inspection again and into the gates are really long....
For me 5 hours are not much and I would stay at the airport... so, this is really up to you.

About 5 miles from the Airport is the Tourist section of SanJuan where all the nice lavish hotels with casinos are.. There is also a beautiful beach right on route 187 which is in the same location. There is a yellow domed stadium which holds about 700 people right in the middle of downtown Sanjuan which holds rooster fights.

Go to the Old San Juan. Is a beautifully city where you can walk, shop and have a good drink. Don't miss El Morro Castle, the Cathedral and the historic colonial streets.

San Juan, PR is a very busy city, you have to make sure you are going to be very close to the airport, because in that city there is always traffic.you can go to Isla Verde and eat good food..but remember don't be late...

hi i travel alot and do work at the airport, you can go to the new casino right at the airport or eat at the caribe restaurant they have real pr food there but the price is kinda high.beware of the beans though jajaja the casino is fairly new so you might win if youtre into that but isla verde is pretty good if you want to venture off like they said keep track of youre time because everything is slow here.
